Insurance, PAYMENT and Reimbursement 

 

Payment: Leslie accepts cash, checks, credit cards, FSA and HSA cards.  Virtual/remote clients are required to maintain a credit card on file for all services (sessions, late cancelled/no show/missed). In-person clients are required to maintain a credit card on file for missed/late cancelled sessions. Payment is expected at the time services are rendered.

 

Leslie is an Out of Network Provider: As a courtesy to her clients, she will submit claims to your insurance company for you. Otherwise, she will provide you with a comprehensive monthly statement so that you can submit claims for reimbursement.

 

Leslie does not accept Medicaid or Medicare.  

 

Reduced Fees: Leslie offers a sliding scale for existing and prospective clients who are experiencing financial hardship and exhibit a financial need. Please contact Leslie to discuss your particular situation.

 

Cancellation Policy: All sessions are by appointment only. Your scheduled appointment time is reserved specifically for you/your family. Although Leslie will attempt to fill cancelled appointment slots (last minute or with advanced notice), it is very difficult to do so on short notice. Therefore, if you do not keep your scheduled appointment or you cancel with less than 24 hours’ advance notice, you will be billed for the full amount of the session; However, if Leslie is able to fill your session time or if a mutually satisfactory time within the same week is determined Leslie will waive the cancellation charge.
